Ministry of Health, Chile wanted to reduce lengthy delays in access to healthcare and rising healthcare costs, driven mainly by the growing burden of chronic disease. The challenge was the urban and rural disparities in access to appropriate, timely care. As a solution ambitious transformational targets was underpinned by explicit service-level agreements for citizens. Self-care healthcare model was designed and implemented for early diagnosis and management of chronic illnesses. There was integration with Chile's national broadband initiative to aggregate demand across the public sector.
