Transcribed clinical notes or commentary can provide useful information to the clinicians who read them. However, from the perspective of data analysis, such notes consist of sequences of individual words assembled in a somewhat orderly fashion that often require human, context related evaluation to be useful. Words may be misspelled, punctuation may be misplaced, individuals may use jargon and local idioms and there may be little consistency of expression among authors. Therefore, a computer program attempting to use these same sequences of words as structured data, ie information, faces a daunting if not impossible task.
